My Health & Wellness Support Benefit
At SOAPassist LLC d/b/a My Health Insight, we believe that understanding your health is an important step toward better health and well-being.
Our Health & Wellness Support Benefit is designed to help eligible members take practical steps toward their health goals following completion of a qualifying health review.
How it works
1. Complete your health review. Participate in a qualifying health review provided by My Health Insight.
2. Confirm your eligibility. Your completed health review and eligibility for the program will be verified according to the applicable program requirements.
3. Receive your support benefit. Eligible members may receive a gift card to help with everyday needs, including groceries and other approved purchases, depending on the available gift-card options.
Benefit limits
Up to $50 per completed qualifying health review.
Up to $100 per member per calendar year.
A maximum of two qualifying health reviews may be eligible for a benefit in a calendar year when the participating health plan determines that a separate additional health review may be warranted because of a suspected significant change in the member’s health.
A second health review and associated benefit are not automatic. The member must meet the applicable eligibility requirements, and the additional review must be authorized under the participating health plan’s program rules.
Available gift-card options
Depending on availability, options may include Walmart, Target, H-E-B, Kroger, Publix, Albertsons, Safeway, Aldi, Whole Foods Market, Sprouts Farmers Market, and other participating national and regional retailers.
The selection of available retailers may vary by location and distribution provider. Not every retailer will be available in every market.
Who funds the benefit?
SOAPassist LLC d/b/a My Health Insight bears the cost of the Health & Wellness Support Benefit. The cost of the benefit is not passed on to or separately charged to the participating health plan.
Important information
The program is subject to participating health plan approval, member eligibility requirements, and applicable legal and regulatory requirements. Completing a health review does not automatically guarantee a benefit.
Participation does not require members to accept a particular diagnosis, treatment, or medical recommendation. Members retain the right to make their own health care decisions in consultation with their health care professionals.
